well that's actually the whole fucking point of the term. They do not do sex testing at birth, they make an educated guess based on external genetilia. That works for most people but its not 100% accurate. it misses plenty of people with ambiguous genitals, or with internal reproductive systems that differ from external, or that have different chromosomes or hormonal balances. Sex is a lot more complicated than most people are educated in, it's not a simple thing. So that language exists for the edge cases, the times when that initial assessment can be wrong.
It's not a lot more complicated. It's very simple. Except in situations where people have a specific medical condition be it an intersex conditions or something else. It's not an educated guess. It's not assigning. It's identifying somebody's biological sex just because something isn't right 100% of the time doesn't mean it's a guess or an assignment of something. It's a lot more accurate than most other tests out there.
it's really not when you have to make a dozen cavates for the instances where it's wrong, which is why the term exists. because in some instances the "observation" of the infants sex, turns out to be wrong.

Those people have a specific medical condition. That's not a normal presentation. It's a result of something not being right genetically.
